If you have been through the referral chain and still do not have a diagnosis, the next step is to schedule a comprehensive intake consultation. This is not a standard evaluation — it is a structured diagnostic workup designed to identify the root cause of your pain.

Your initial consultation spans 60 to 90 minutes and includes a complete medical and dental history review, comprehensive cranial nerve examination, jaw range-of-motion testing, intraoral assessment, and diagnostic imaging when indicated.

Your First Visit.

01Schedule Your Appointment

Call our office or send an email to request an intake consultation. Our team will confirm your appointment and provide any preparation instructions.

02Complete the Intake Questionnaire

Before your visit, complete our comprehensive intake questionnaire. This provides the clinical team with your pain history, prior treatments, and imaging records in advance.

03Diagnostic Consultation (60–90 Minutes)

Your initial visit includes a complete medical and dental history review, comprehensive cranial nerve examination, jaw range-of-motion testing, intraoral assessment, and diagnostic imaging when indicated.

04Preliminary Diagnosis and Next Steps

You will leave with a preliminary differential diagnosis and a clear understanding of the next steps in the diagnostic process. Additional testing is scheduled as needed.
